Home Tele-Health is available for Veterans at the Fargo VA Healthcare System.

 

Home Tele-Health is a program that uses equipment connected to your home telephone to send your daily health reports to a nurse at the Fargo VA. Tele-Health is available for Veterans with heart failure, lung disease, kidney disease, high blood pressure and/or diabetes.

What does it do?

Once it is set up in your home, the equipment will:

  • Allow closer monitoring of your chronic health conditions.
  • Help your provider to quickly change your medications and treatments
  • Provide timely health care advice
  • Help you take better care of yourself
  • Lead to fewer complaints, hospital admissions, urgent care and emergency visits
  • Improve your quality of life and give your family more information
  • Can extend the time you are able to stay in your home

How does it work?

  • The equipment connects to your regular phone line. You will be able to use your phone as usual and the phone bill will not change
  • You will be shown how to use the equipment prior to taking it home or may be instructed on use by a home health nurse or over the phone
  • It is easy to use- just push a button to turn it on!

  • Home Tele-Health is NOT an emergency response system and it does not replace all clinic or home visits.
